To qualify, you must have Medicaid and fit into one of these groups:
- 65 and older
- A child or adult with disabilities
- Receiving long-term services and supports
- A nursing facility resident
Note: If any of these describe you, the state will automatically enroll you into CCC Plus.

The steps below will help you understand how to apply for Medicaid in Virginia and get care through Kaiser Permanente for yourself or your child. Open All + 1. See if you and/or your family may qualify for Medicaid.
2. Find out if Medicaid through Virginia Premier with Kaiser Permanente is available in your area.
3. Complete your Medicaid application.
4. Receive approval for Medicaid.
After you’ve been approved for Medicaid, here’s what you need to do to select Medicaid with Kaiser Permanente:
5. Choose Virginia Premier with Kaiser Permanente as your Managed Care Organization.
6. Receive a new-member packet from Virginia Premier and Kaiser Permanente once you’re enrolled.

- You may also click here (www.wvpath.org) to apply online for Medicaid and other DHHR programs.
- You may go to your local DHHR office. Click here to find your local office.
- Fill out a paper application (Links below) and send it to your local DHHR office.
- Call the Customer Service Center at 1-877-716-1212 to apply over the phone.

If You Receive Supplemental Security Income (SSI)
If you are receiving a check from Supplemental Security Income Program, you are automatically eligible for Medicaid and should receive a medical card from the West Virginia Department of Health and Human Resources.
If
You DO NOT Receive SSI
If you are not receiving an SSI check, you may apply for Medicaid benefits using any of the options above. Applications are taken weekdays at your local office of the West Virginia Department of Health and Human Resources.
Most local hospitals and primary care clinics have staff available to assist you in making application.
If, because of a physical handicap or disability, you are unable to go to the local office, you may request a staff person to visit your home and take the application. To request a home visit, call your local office of the West Virginia Department of Health and Human Resources or call the Office of Client Services toll free at (800) 642-8589.
