REQUIREMENTS

• Personal appearance is required for application at the Consulate or during the Consular Outreach

• Do not bring passport pictures. Your photo and other biometrics are to be taken by the Consulate staff.

• When going to the Consulate or Consular outreach, make sure you wear decent clothes and upper garments with sleeves and collar.

• Just before your photo and biometrics are taken by the Consulate staff, please remove your eyeglasses/contact lenses, necklace, earrings, and when applicable, facial piercings.

• A Philippine passport remains at all times the property of the Philippine Government, the passport holder being a mere possessor. Hence, the loss, mutilation or destruction of the Philippine Passport constitutes an offense with penalties set by law.

• The Consular Officer reserves the following rights: (1) right to require additional proof or documents, pursuant to the Philippine Passport Law and the Foreign Service Act and (2) right to refuse serving clients who fail to observe proper office decorum and health standards.

1. 1. Confirmed Passport Appointment.

   2. Passport Application Form [PDF]filled up and signed by applicant

   3. Report of Birth (For those under our jurisdiction see Report of Birth Requirements) signed by Philippine Consular Officer;

      • IF Report of Birth was registered one (1) year or more from date of passport application, bring original and photocopy of the the Philippine Statistics Authority (PSA) (formerly National Statistics Office or NSO) Report of Birth [Apply here for PSA Birth certificate];

   4. Valid Identification Cards such as US Permanent Resident Card or State Identification Cards;

   5. Other supporting documents which will establish the applicant’s identity as a Filipino citizen, i.e. marriage certificate, school records, professional ID, etc;

   6. For applicants who are military servicemen/military personnel and who came to the U.S. using only military ID and without a Philippine passport, a Certification (with duplicate) issued by his/her Commanding Officer to the effect that he/she is still a Filipino citizen who is on active duty with the U.S. military service must be submitted. In addition, applicants falling under this category must submit: original or authenticated copy of birth certificate issued by PSA/NSO [Apply here for PSA Birth certificate]

   7. For adopted adults: In addition to requirements above, the applicant must bring the following documents:

      • original and photocopy of PSA Birth Certificate issued after adoption decree [APPLY HERE];

      • Certified true copy of the adoption decree issued by a Philippine court or foreign adoption decree recognized by a Philippine court;

6. For applicant using husband’s surname for the first time: In addition to requirements in items 1-5, the applicant must bring the following documents:

   • In case of woman married in the U.S.:

     • If Report of Marriage (ROM) was registered less than year from date of passport application: original and photocopy of the Report of Marriage (ROM); (For those under our jurisdiction, see Report of Marriage Requirements)

     • If ROM was registered one (1) year or more from date of passport application: bring original and photocopy of PSA ROM.

   • In case of woman married in the Philippines or country other than the U.S.:  original and photocopy of Marriage Contract duly authenticated by the PSA [apply here for your PSA Marriage Certificate]

7. For a married woman wanting to revert to her maiden name after Husband’s death or legal dissolution of marriage: In addition to the requirements in items 1-5, the applicant must bring:

   • For widowed applicants – original and a photocopy of the husband’s PSA death certificate or PSA issued marriage certificate with annotation of court decree of declaring husband’s presumptive death. [apply here for your PSA Certificate]

   • For annulled marriages – original and photocopy of annotated PSA Marriage Certificate with annotation indicating that the Philippine Court issued an annulment decree or decree of nullity.[apply here for your PSA Certificate]

   • For divorced applicants: original and photocopy of annotated PSA Marriage Certificate with annotation indicating that the divorce has already been recognized by a Philippine Court. [apply here for your PSA Certificate]

8. For applicants who are military servicemen/military personnel and who came to the U.S. using only military ID and without a Philippine passport, a Certification (with duplicate) issued by his/her Commanding Officer to the effect that he/she is still a Filipino citizen who is on active duty with the U.S. military service must be submitted. In addition, applicants falling under this category must submit: original or authenticated copy of birth certificate issued by the PSA/NSO [Apply here for PSA Birth certificate];

9. For adopted adults: In addition to requirements above, the applicant must bring the following documents:

   • original and photocopy of PSA Birth Certificate issued after adoption decree [APPLY HERE];

   • Certified true copy of the adoption decree issued by a Philippine court or foreign adoption decree recognized by a Philippine court;

3. If born outside the Philippines: Report of Birth (For those under our jurisdiction, see Report of Birth Requirements) signed by Philippine consular officer;

   • IF Report of Birth was registered one (1) year or more from date of passport application, bring original and photocopy of the Philippine Statistics Authority (PSA) (formerly National Statistics Office or NSO) Report of Birth [Apply here for PSA Birth certificate]

4. If born in the Philippines: PSA/NSO Birth certificate [Apply here for PSA Birth certificate]

5. For legitimate minors: In addition to the requirements in items 1-2, 3 or 4, personal appearance of either parent is required during the application. If both parents are unable to appear, then the legal guardian must appear with the minor and bring the following documents:

   • Affidavit of Support and Consent to Travel [click here for pdf ] executed by either parent indicating the name of child’s travelling companion.

   • Special Power of Attorney with an attached copy of either parent’s valid passport authorizing a representative to assist the child in the application.

   • Original and photocopy of the valid Identification card of the parent’s representative.

   • Photocopy of the valid PH passport or valid ID of either parent.

   • Original and Photocopy of NSO/PSA Marriage Certificate of Parents

6. For illegitimate minors: In addition to the requirements in items 1-2, 3 or 4, personal appearance of  the mother is required during the application. The mother must bring with the minor the following documents:

   • Original and photocopy of her valid PH passport or valid identification card.

7. For illegitimate minors acknowledged by the father: In addition to the requirements in items 1-2, and 6, the mother must bring the minor and the following applicable document:

   • If Report of Birth was registered less than one (1) year from date of passport application: the Report of Birth showing the child using father’s surname.

   • If the Report of Birth was registered one (1) year or more from date of passport application: original and photocopy of the PSA birth certificate reflecting the father’s surname with the father’s Affidavit of Acknowledgement and Consent to use his surname [APPLY HERE].

8. For minors legitimated by the subsequent marriage of parents: In addition to the requirements in items 1-2, and 5, either parent must bring the minor and the following applicable document:

   • If Report of Birth was registered less than one (1) year from date of passport application: the Report of Birth showing the child using father’s surname and annotation that child’s status as legitimated.

   • If the Report of Birth was registered one (1) year or more from date of passport application: original and photocopy of the annotated PSA birth certificate on the child’s status as legitimated

   • If born in the Philippines: original and photocopy of the PSA birth certificate with the annotation of the child’s legitimated status.

   • If both parents are unable to appear, then the legal guardian must appear with the minor and bring the following documents:

     1. Affidavit of Support and Consent to Travel [click here for pdf ] executed by either parent indicating the name of child’s travelling companion.

     2. Special Power of Attorney with an attached copy of either parent’s valid passport authorizing a representative to assist the child in the application.

     3. Original and photocopy of the valid Identification card of the parent’s representative.

     4. Photocopy of the valid PH passport or valid ID of either parent.

     5. Original and Photocopy of NSO/PSA Marriage Certificate of Parents

9. For adopted minors: In addition to the requirements in items 1 and 2, the adoptive parent/s must appear and bring the following documents:

   • original and photocopy of PSA Birth Certificate issued after adoption decree [APPLY HERE];

   • Certified true copy of the adoption decree issued by a Philippine court or foreign adoption decree recognized by a Philippine court;

10. For minors who are dual citizens by birth and born in the Philippines: In addition to the requirements 1,2 and 4. Consular Report of Birth issued by the Embassy of the country of the foreign parent (i.e. Consular Report of Birth issued by the U.S. Embassy in Manila if the minor is born of a Filipino mother and an American father).

OTHER INFORMATION

• Processing Fee: $60.00 in cash, US Postal Money Order or cashier’s check payable to the Consulate.   

• All passports are printed in the Philippines. The estimated time of your passport’s arrival at the Consulate is six (6) to eight (8) weeks from date of processing. 

• For Releasing of Passports :

  • For passports processed after 19 March 2020 the only option is to mail your new passports.

    • Bring your own United States Postal Service (USPS) Priority Mail/Priority Mail Express Envelope. The USPS Priority Mail Envelope has to be self-addressed, postage prepaid and with USPS Tracking. To compute the amount of postage for a USPS Flat Rate Envelope, check here.

    • The Consulate shall not assume any responsibility for passports/documents mailed through USPS . 

  • For those whose passports were processed before 19 March 2020 and chose to pickup:

    • For adult applicant, please present:

      1. Your official receipt.

      2. If your old passport has not been cancelled, please bring and present the passport.

         • If the un-cancelled old passport was lost, bring and present PSA/NSO Birth Certificate, Marriage Certificate (if married) and a Notarized Affidavit of Loss

• For representative of adult applicant, please present:

  • Applicant’s Official Receipt.

  • Authorization letter if the authorized representative is immediate family member – spouse, father, mother, children who are at least 18 years old

  • Notarized Special Power of Attorney if the authorized rep is not immediate family member – aunts, uncles, in-laws, grandparents, friends, neighbors, officemates, etc.

  • Copy of valid ID of the passport applicant

  • Copy of valid ID of the authorized representative

  • If applicant’s old passport for has not been cancelled, please bring and present the passport.

  • If the un-cancelled old passport was lost, bring and present applicant’s PSA/NSO Birth Certificate, Marriage Certificate (if married) and a Notarized Affidavit of Loss

• For minor applicant:

  • If applicant’s mother will claim the passport, please present:

1. 1. 1. 1. Official receipt issued to the applicant

         2. Copy of applicant’s birth certificate

         3. If applicant’s old passport for has not been cancelled, please bring and present the passport.

            • If the un-cancelled old passport was lost, bring and present applicant’s  Birth Certificate and a Notarized Affidavit of Loss

• • If applicant’s father will claim the passport, please present:

    1. 1. 1. Official  receipt issued to the applicant

          2. Applicant’s birth certificate

          3. If the parents are not married, authorization letter from the mother with the mother’s ID are required.

          4. If applicant’s old passport for has not been cancelled, please bring and present the passport.

             • If the un-cancelled old passport was lost, bring and present applicant’s PSA/NSO Birth Certificate and a Notarized Affidavit of Loss

• • If the sibling who is at least 18 years old will claim the passport, please present:

    1. 1. 1. Official  receipt issued to the applicant

          2. Authorization letter from father or mother with their valid IDs if parents are married

          3. Authorization letter and valid ID of the mother, if parents are not married

          4. If applicant’s old passport for has not been cancelled, please bring and present the passport.

             • If the un-cancelled old passport was lost, bring and present applicant’s  Birth Certificate and a Notarized Affidavit of Loss
